Get Started with an Evaluation

We are a pediatric neuropsychology clinic, which means that we provide evaluations for individuals ages 4-22 with various conditions.

We are not currently offering Autism diagnostic services.

If you would like your medical insurance to help cover the cost of your appointment(s), please have your provider fax us a referral form at 855-702-2520. Our referral form can be downloaded here.

Once the referral is faxed, please get in touch with our office to confirm receipt. Please allow us 1-2 weeks to review it, after which we will contact you to proceed with the intake process.

Depending on the current volume of referrals, we may be able to schedule you; however, we may need to add you to our waitlist.
